RECOMMENDATION

Approve and authorize the Chairman to sign an agreement with ACE Interface, LLC, Laura Porter and Robert Anda, MD, MS, Co-Founders, in an amount not to exceed $86,200 to provide workshops and trainings regarding effective application of the Adverse Childhood Experience (ACE) study and a limited license from ACE Interface to use the training materials effective the date of signing through June 30, 2020.

SUMMARY

Approval of the recommendations will allow the Health and Human Services Agency (HHSA)-Public Health Branch to contract with ACE Interface to provide community workshops and conduct the Train the Master Trainer Program in Shasta County.


DISCUSSION

Adverse Childhood Experiences (ACEs) are the negative occurrences in childhood, including abuse, neglect or household dysfunction, which are linked to poor physical and mental health outcome in adulthood. Shasta County residents have a high prevalence of ACEs, approximately twice the state average. A survey was conducted with Shasta County residents between the ages of 18 and 64 in spring 2012. The responses given by Shasta County residents revealed significantly higher rates of adverse childhood experiences (ACEs) when compared to a National Survey completed in 2009 of five states. In Shasta County, of the 281 Shasta County residents who answered the survey questions, 29% of respondents reported five or more ACEs in their life history compared with just 9% in other parts of the country, while 16% of Shasta County residents report no ACEs in their history, compared to 41% elsewhere. As such, the HHSA-Public Health Branch proposes to partner with the Strengthening Families Collaborative and work with ACE Interface, LLC to meet the HHSA Strategic Plan Priority to Leverage Strategic Partnerships and Reduce Adverse Childhood Experiences through workshops and support to create a Three Year Community ACE Prevention Plan, a data dashboard to monitor health outcomes associated with this plan, and train 25 key community partners as Master Trainers through the ACE Interface evidence-based program.

The ACE Interface Train the Master Trainer Program is designed to support the dissemination of the science related to ACEs and, and promote application of the science to improve health and wellbeing across the lifespan.  The program is structured with licensed materials, a two-day training provided by Dr. Robert Anda and Laura Porter, and a process for effectively implementing a comprehensive ACE education campaign across sectors based on person-to-person interaction and community engagement. Technical support and updating all materials to align with new research in this area is included for a 3 year period. Dr. Anda was the Centers for Disease Control researcher that worked with Dr. Felleti in a study done at Kaiser Permanente-San Diego that originally identified the ten ACEs between 1995 and 1997. Dr. Anda and Laura Porter are nationally recognized experts in the field of ACEs and working with communities to break generational cycles for improved health outcomes.


ALTERNATIVES

The Board could choose not to allow HHSA Public Health to contract with ACE Interface, LLC, may request changes to the agreement, or provide alternate direction to staff.


OTHER AGENCY INVOLVEMENT

Shasta County HHSA-Public Health Branch is member of and will collaborate with the Strengthening Families Collaborative which consists of 30 county-wide partners dedicated to making an impact on our community. The Strengthening Families Collaborative steering committee includes representatives from non-profit community based organizations, education, Probation, early learning, child abuse prevention, youth violence prevention, and intimate partner violence prevention. ACE prevention is included in the Community Health Improvement Plan of Shasta County and the HHSA Strategic Plan.

The agreement has been approved by Risk Management and approved as to form by County Counsel. The County Administrative Office has reviewed the recommendation.


FINANCING

There are sufficient appropriations included in the HHSA – Public Health Fiscal Year 2016-17 Adjusted Budget.  Associated revenues and expenditures will be included in the Fiscal Year 2017-18 HHSA Public Health Requested Budget.
