The California Department of Health Care Services (“DHCS”) established the Housing and Homelessness Incentive Program (“HHIP”) to improve health outcomes and access to whole person care services by addressing housing insecurity and instability as a social determinant of health for the Medi-Cal population. HHIP is designed to develop capacity and partnerships to connect members to needed housing services and reduce and prevent homelessness. HHIP is a Medi-Cal Managed Care Plan (“MCP”) and PHC is a HHIP participating MCP.
PHC as a MCP will be eligible to obtain up to $22 million in HHIP payments for NorCal Continuum of Care (“CoC”) members as well as earn PHC incentive payments for itself, however, in order to obtain the funds and incentive payments from DHCS, DHCS requires PHC to enter into a two-way data sharing agreement with Shasta County, as the NorCal Lead Agency operating the CoC Housing Management Information System (“HMIS”).
The County, through its Department of Housing and Community Action and Health and Human Services Agency and PHC agree to work together to facilitate data sharing pursuant to the proposed agreement, in order for PHC to receive funding to pass to other CoC members.
